Alexandra Daddario has been one of Hollywood’s most recognizable faces for over a decade, from Percy Jackson to Baywatch to her Emmy-nominated turn on The White Lotus. But her personal life took a quieter, more intimate turn during the COVID-19 pandemic, when she met film producer Andrew Form on a nearly empty New York City street. What followed was a whirlwind romance, a New Orleans wedding, a baby, and — after just over three years of marriage — a separation.
Here’s what we know about Alexandra Daddario’s husband, Andrew Form, their relationship, and how it came to an end.
Who Is Andrew Form?
Andrew Form is a veteran Hollywood film producer born on February 3, 1969. He is 57 years old and originally from New York.
Form began his career as a production assistant for Jerry Bruckheimer before co-founding Platinum Dunes in 2001 alongside Michael Bay and Brad Fuller. The company became known for producing reboots of classic horror films including The Texas Chainsaw Massacre, The Amityville Horror, Friday the 13th, and A Nightmare on Elm Street.
He is perhaps best known for producing the A Quiet Place franchise, The Purge series, and Teenage Mutant Ninja Turtles. In 2015, The Hollywood Reporter named him one of the “30 Most Powerful Film Producers in Hollywood.”
Before his relationship with Daddario, Form was married to actress Jordana Brewster for nearly 14 years. They met on the set of The Texas Chainsaw Massacre: The Beginning and married in 2007. They have two sons together — Rowan, now 9, and Julian, now 12 — and finalized their divorce in June 2021.
How Alexandra Daddario and Andrew Form Met
Their love story began during the COVID-19 lockdowns in New York City — a time when streets that are normally packed with people were eerily empty.
Daddario recounted the moment to Vogue in June 2022. She was staying on an acquaintance’s couch while waiting for her co-op to be approved and took frequent walks to pass the time. “He was also on a walk, and we passed each other downtown,” she said. “He turned back and said ‘hi’ just as I was turning around to glance back at him. I laughed and said ‘hi,’ and then we both laughed about how we were the only two people on what is normally an incredibly busy street, and he asked if he could take me to dinner.”
She said yes — against what she jokingly called her better judgment, noting that “nowadays you’re supposed to meet people on dating apps.”
Their first date took place in Form’s hotel room at the Greenwich Hotel. Because of COVID restrictions, the hotel was nearly empty — only three guests in the entire building. They had dinner by the fire in the downstairs living room, and Daddario brought her dog. At one point, the dog wandered into the kitchen without them noticing. “Someone came out holding her, and said, ‘Is this your dog?’” Daddario recalled.
The couple made their relationship Instagram official in May 2021 with a kissing photo. Their engagement was confirmed in December 2021.
The Wedding: A Vintage Affair in New Orleans
Alexandra Daddario and Andrew Form married in June 2022 at Preservation Hall in New Orleans. The venue was intimate and vintage-themed, a fitting choice for a couple who seemed to value authenticity over spectacle.
Daddario wore a long pleated dress with a trailing tulle veil by designer Danielle Frankel, while Form wore a lightweight pinstripe suit by Brunello Cucinelli. In photos shared on Instagram, the couple was seen kissing on the streets of Louisiana, holding white and black umbrellas.
Form’s two sons from his previous marriage, Julian and Rowan, served as groomsmen and ring bearers, and guests followed a second line parade to Bar Marilou for the reception.
Daddario later said on the Today show that what drew her to Form was simple: “He’s just a really wonderful person and very funny and it just clicks.” She also emphasized that she had always wanted to be a mom and get married from a young age. “I’ve never been scared of monogamy or settling down or anything like that. It just clicked with Andrew.”
Welcoming Their Son and the Joy of Motherhood
In July 2024, Daddario announced she was pregnant with the couple’s first child. In an interview with Vogue, she revealed she had previously experienced a pregnancy loss. “Well, I had been pregnant,” she said. “I had loss. It’s long and complicated, so I don’t want to be too specific. Those kinds of losses and trauma are very hard to explain unless you’ve been through them.”
Their son was born on October 31, 2024 — Halloween. In February 2025, Daddario told People that motherhood had been “wonderful.” “I’m very lucky,” she said. “He’s a great baby. He’s just brought so much joy and peace in my life, so I recommend it to anyone who wants a kid.”
In her interview with Elle in late 2025, Daddario reflected on how becoming a mother had deepened her approach to acting and life. Discussing her role in I Wish You All the Best, she said, “I wanted to be a mother for a very long time, and I felt that there was something I really related to in the role that really drew me to her. I wasn’t a mother at the time, and now I am, but I think being that person that can guide younger people through growing up, while staying calm and trying to figure everything out and just be that light to the extent that they can be — that’s what I want to be for my child.”
The Separation: Ending the Marriage After Three Years
On February 20, 2026, a representative for Alexandra Daddario confirmed that she and Andrew Form had decided to end their marriage after more than three years together.
“Alexandra Daddario and Andrew Form have made the decision to end their marriage,” the statement read. “The decision was made with love and respect. They will continue to co-parent their child together and appreciate privacy as they navigate this transition.”
According to court records, Daddario filed for divorce on Friday, February 20, 2026. The couple’s son was 15 months old at the time of the announcement.
The two were last seen together on a red carpet in September 2025 at The Kids Mental Health Foundation LA Committee Dinner and Celebration in Los Angeles.
What’s Next for Alexandra Daddario
Since the separation, Daddario has continued to focus on her career and her son. In September 2026, she shared vacation photos in a gingham bikini, looking relaxed and confident seven months after the split announcement.
Professionally, she has been busy filming the third season of AMC’s Mayfair Witches in Vancouver, where she plays a neurosurgeon who is also a witch. She also starred in the 2025 indie film I Wish You All the Best, directed by Tommy Dorfman, and remains a fan-favorite candidate for superhero roles in James Gunn’s DC Universe — though she told ScreenRant she was unaware of the viral campaign to cast her as Wonder Woman.
For Daddario, the end of her marriage marks a new chapter. She has spoken openly about wanting partnership and motherhood, and she appears to be navigating this transition with the same grounded perspective she brought to her relationship from the start — one built on a chance meeting, a shared laugh, and the belief that connection can happen when you least expect it.
